Abstract
Background:
@EN Congestive heart failure is a serious and growing clinical problem. Diuretics and digitalis glycosides remain the traditional regiments of therapy in patients with congestive heart failure, however, angiotensin converting enzyme(ACE)
inhibitors
have
been added as an important treatment option. Perindopril, a new second generation ACE inhibitor, was added to the patients with severe chronic congestive heart failure who were unresponsive to diuretics and digitalis glycosides to evaluate the
clinical
and echocardiographic findings.
@ES Method:
@EN The changes of clinical symptoms and signs, echocardiographic findings serum biochemical examination. ECG, chest X-ray and side effects were evaluated before and 6 months after addition of 2-4mg perindopril in the patients with severe chronic
congestive heart failure who were unresponsive to diuretics and digitalis glycosides. A total of 63 patients with a man age 57.4¡¾8.7 years. CHF of New York Heart Association(NYHA) more than class III and below 30% of ejection fraction were
included
after treatment with diuretics and digitalis glycosides more than 3 years.
@ES Results:
@EN 1) The clinical symptoms of dyspnea. Hepatomegaly and pitting edema were significantly improved after the addition of perindopril.
2) The cardiomegaly, lung congestion on chest X-ray, elevated level of serum blood urea nitrogen, aspartate aminotransferase and alanine aminotransferase were significantly improved after the addition of perindopril
3) In echocardiographic findings, the left ventricular end systolic dimension (55.5¡¾4.8¡æ47.6¡¾4.7mm), the left ventricular end systolic volume (140.4¡¾28.7¡æ82.4¡¾6.4ml), interventricular septal thickness (13.8¡¾3.2¡æ11.6¡¾1.7mm), frectional
shortening (18.5¡¾3.4¡¾25.7¡æ3.4%) and ejection fraction(25.1¡¾3.8¡¾38.1¡æ3.2%) were significantly changed.
4) The side effects were 11 cases of dry cough, 6 headache, 4 hypotension, etc.
@ES Conclusion:
@ES Perindopril had been shown to shown to be an extremely useful agent in the management of the patients with severe chronic heart failure who were unresponsive to diuretics and digitalis glycosides.
